Service Description: Integrated day program for frail elderly, adults with special needs and individuals with Alzheimer Disease or other dementia.

Day program for adults who require some physical, social or emotional support to remain in the community.


Fees: Program - $16 per day
Transportation available through DARTS and Red Cross at an additional fee
Application: Apply through the Hamilton Niagara Haldimand Brant Community Care Access Centre (HNHB-CCAC)
Eligibility - Population(s) Served: Age 18 years and older * must meet Hamilton Niagara Haldimand Brant Community Care Access Centre (HNHB-CCAC) criteria
