Service Description: The Assertive Outreach Team is a multi-disciplinary team that engages with individuals who are living with mental illnesses and are living in shelters or are homeless.

The case Intensive Case Management team is staffed by Public Health nurses who work with adults who are diagnosed as living with a serious mental illness, to promote the achievement of optimal health and well-being and to increase independence and participation in community life.

This program does not provide mental crisis services. Individuals in crisis should call COAST at 905-972-8338.
