Service Description: Outpatient rehabilitation program for people recovering from cardiac events, or who may be at high risk for a cardiac event * services include:
  • nutrition counselling and nutrition education classes
  • exercise counselling and supervised exercise classes
  • group stress management classes
  • smoking cessation support
  • heart health education

Healthcare team includes a physician, registered nurse, registered practical nurse, dietitian, social workers, and kinesiologist

Maintenance level of rehabilitative care for patients working to prevent functional decline/injury or maintain functional performance

Fees: Covered by OHIP
Application: Medical referral required
Eligibility / Target Population: People recovering from cardiac events, or who may be at high risk for a cardiac event
