Service Description:

Telephone referral service for people to call to help connect an at-risk older adult with services to help them stay safe and independent in their home * older adults are referred to partnering organizations that can best meet their needs * services are provided with the consent of older adult * information is kept confidential and callers may remain anonymous

Adults 60 years and older * has or is eligible for a valid Ontario Health Card * resides in the community within Niagara Region
