Service Description:
|
Provides support for high-risk transitions to long-term care homes for people who exhibit responsive behaviours * transitional lead works with client's family and care providers to develop behavioural care plans as client is admitted into long-term care * works with BSO Long-Term Care Home Mobile Team to provide intensive support in first six weeks after admission

Older adults with cognitive impairment who exhibit, or are at risk of exhibiting responsive or challenging behaviours.
