Service Description: Clinical outpatient program for youth with emerging mental health or substance use issues

Transitional Support
- Provides transitional supports and system navigation to youth 16 to 19 years with mental health and/or substance use issues who are moving into the adult system

Assessment and Treatment
- Provides youth 16 to 25 years with comprehensive assessment and treatment for untreated mental health and/or substance use issues

Fees: Most services covered by OHIP
Application: Transitional Support: Referral required from a professional at one of the hospital's community child and adolescent partners
Assessment and Treatment: Self-referral or referral from a relative, friend, physician, or other professional
Eligibility / Target Population: Youth who live in Burlington and have mental health and/or substance use issues
