Service Description: 10-bed hospital inpatient rehabilitation program for patients who reside in Brant Haldimand Norfolk, who are medically stable, and have been diagnosed with a stroke * patients will have established functional goals to support a return home * maximum length of program is 46 days and based on the patients ongoing progress

Fees: Covered by OHIP
Application: Medical referral required
Eligibility / Target Population: People in need of stroke rehabilitation following inpatient acute care
