Service Description: Supports for older adults, and adults with physical disabilities and mental health concerns * helps people remain independent in their own homes for as long as possible
* provides care and support to people who are unable to care for themselves: dementia care, home care, social support, fall prevention, hospice, end of life care and bereavement support * programs to support both the physical and mental wellbeing of people by reducing social isolation and providing care: adult day programs, support groups, nursing care, and friendly visiting programs
 
Community Support Services - Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible
 
Patty's Place - Dementia Care Centre - Supports for people living with dementia: adult day programs, overnight respite, caregiver support and education
 
Technology Accessibility Program - The program helps seniors to stay connected with friends and family by using technology * participants will receive individual training in their own home, once a week for three months * training is provided by volunteers * tablets are available on loan and access to the internet can be provided, if required

Application: Referral required for some programs
Eligibility / Target Population: Older adults and people with disabilities, who need physical, social, or emotional supports
