Medical and non-medical assistance for adults with disabilities discharged from hospital to manage the activities of daily living in an apartment setting * services are provided on a scheduled and "on-call" basis
- bathing, dressing, grooming
- medication monitoring
- laundry
- light housekeeping
- meal preparation
- mobility assistance
- nursing
- security checks
- shopping and grocery assistance

Eligibility / Target Population:
|
Adults 18 years and older with physical disabilities who are discharged from hospital who need personal care and nursing assistance for the activities of daily living * must be medically stable * must be able to direct and participate in own care
