Service Description:

Mobile outreach service for people who have frequent visits to the emergency department for mental health and/or addictions concerns * multidisciplinary team helps clients navigate healthcare system and connects them to community agencies for primary healthcare and mental health and addictions treatment
Program run in collaboration with CMHA Niagara, Community Addiction Services of Niagara, Niagara Health, and Segue Clinic

People who make frequent visits to the emergency department for mental health and/or addiction concerns * people with an opioid dependency issue (current or within the last 12 months)
