Service Description:
|
Free community support services to promote successful discharges from either in-patient hospital stays or emergency departments * provides individuals to access services for a 4-6 week period to regain strength and independence * services may include:
- up to 12 frozen meals
- transportation home from hospital
- transportation for 2 follow up medical appointments
- homemaking services
- home safety assessment
- communication with other support services

Eligibility / Target Population:

Adults 60 years and older who are being discharged from hospital * Must be medically stable and able to transfer independently
