Service Description:
|
No charge community support services to promote successful discharges from either in-patient hospital stays or emergency departments * provides individuals to access services for a 6 week period to regain strength and independence * services include:
- hot or frozen meal delivery
- transportation for 3 follow up medical appointments
- homemaking services - 3 visits within 6 weeks
- home fall safety assessment
- weekly wellness check ins for isolated referrals
- service navigation
- respite (for patients with cognitive decline/diagnosed dementia)

Eligibility / Target Population:

Older adults with function or cognitive impairment being discharged from hospital * must be medically stable and able to transfer independently
