Service Description: Provides short-term behavioural support and assistance for clients in crisis or at risk of a crisis and their families and help them connect with appropriate longer-term services and community resources * program is offered through a collaboration of Local Health Integration Network (LHIN), local Crisis team, and Alzheimer Society

Support and assistance include:
  • crisis intervention
  • practical support and outreach
  • education
  • advocacy
  • coordination of services
