Service Description: Adult day program * social, recreational, and therapeutic activities for older adults, adults with Alzheimer's disease and other related dementia, and adults with disabilities in a group setting for part of the day * helps to support family caregivers who provide day-to-day care

Programs are designed to meet a variety of needs and interests and assist people to achieve and maintain their maximum level of functioning, postpone or prevent institutionalization

Services include:
  • respite and support for family and caregivers
  • transportation (additional cost)
  • nutritious meal and refreshments
  • medication reminders
  • personal care assistance
  • daily exercise program

Fees: $19 per day
Application: Apply through the Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB-LHIN)
Eligibility / Target Population: Must meet Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB-LHIN) criteria
For adults who are trying to remain in independent living in the community and may be experiencing one or more of the following conditions: frailty/social isolation; a progressive cognitive disorder such as Alzheimer's disease or other dementia; and an aqcuired brain injury or chronic health condition
