Rehabilitative Care - In-Home - Haldimand Norfolk

Rehabilitation services provided in the patient's home or other non-clinical locations. Services can include physiotherapy, occupational therapy, speech language pathology, case management, and vocational or other evaluations.

Publicly funded in-home rehabilitation services may be arranged by Ontario Health atHome.
 
These services are located outside of Haldimand Norfolk, but provide service to Haldimand Norfolk.
