05 - Institutional Risk - Hamilton Niagara Haldimand Brant


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP identifies people who have an increased risk of moving into a long-term care home in the near future. The goal is to avoid premature admission to a long-term care home by supporting family efforts and offering community intervention programs.
