08 - Communication - Brant


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on the connection between the person's communication abilities and their cognitive skills for daily decision-making. The goal is to work with the person to improve their communication ability whenever possible and to prevent avoidable communication decline.
