23 - Tobacco and Alcohol Use - Burlington

Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on the consumption of alcohol and use of tobacco. Assessors ensure people who smoke or consume high levels of alcohol get appropriate advice and support to either quit or reduce their use of these substances.

See Alcohol and Drug Addiction Assessment and Treatment and Smoking Cessation for treatment and support options for people who trigger this CAP.
