06 - Cognitive Loss - Burlington
Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.
This CAP focuses on helping people with reasonable cognitive skills stay independent as possible for as long as possible. The goals are to improve ability to carry out activities of daily living, prevent further cognitive and physical decline, and support safe and independent decision-making.
