09 - Mood - Hamilton Niagara Haldimand Brant


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on identifying depression, with a pre-existing diagnosis or a depressed mood state that needs attention or possible diagnosis. Assessors work to determine the cause of the mood disorder, identify an appropriate treatment, and monitor the treatment.
