11 - Abusive Relationship - Hamilton Niagara Haldimand Brant


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP identifies people who are in situations of possible abuse or neglect, and to help in the decisions for action. The short-term goal is to determine whether the situation requires immediate action (interventions include social services, medical care, court orders for protection, or relocation of the person), while the medium- to long-term goal is to manage psychological consequences arising from a history of abuse.
