13 - Social Relationship - Burlington

Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP identifies people with declining levels of social exchange. Assessors look at factors associated with reduced socialization and address interventions to increase social engagement.
