18 - Undernutrition - Niagara North West


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered.  The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on people who rank below their “medically recommended” ideal body weight and ensure they get proper nutrition. Assessors will review any underlying diseases, conditions, or medications that contribute to undernutrition or the risk of undernutrition.
