16 - Pressure Ulcers - Hamilton

Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on the prevention of pressure ulcers and the treatment of present ones. Assessors will explore complicating factors if an ulcer does not show signs of healing.
