07 - Delirium - Hamilton Niagara Haldimand Brant


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on issues of delirium (acute cognitive loss) and any related diagnoses (chronic cognitive loss or dementia). The goal of treatment is to return the person with active symptoms of delirium to their baseline status.
