17 - Cardio-Respiratory Conditions - Brant

Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP alerts the health care professional if the person needs to be assessed and managed for possible cardiovascular or respiratory issues.

See Heart Disease, Lung and Respiratory Conditions, Oxygen and Respiratory Services, and Smoking Cessation for care options for people who trigger this CAP.
