17 - Cardio-Respiratory Conditions - Burlington

Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP alerts the health care professional if the person needs to be assessed and managed for possible cardiovascular or respiratory issues.
