Transitional Care Beds - Niagara

Transitional Care Bed (TCB) program is available to eligible patients who require an enhanced level of Personal Support for a short period of time (30-60 days) while they determine their future living arrangements. These future living arrangements may or may not include Long-Term Care, returning home or an assisted living setting.
