10-week program for people who have Chronic Obstructive Pulmonary Disease (COPD), including those recently hospitalized due to COPD

- caregiver support
- group and individual COPD education
- personalized exercise programs
- personal counselling and spiritual support
- smoking cessation counselling and support
Progression level of rehabilitative care for patients working to restore or maximize functionality
