Service Description:
|
On-site respite house
- provides short-term, 24/7 care and support for children and adults with developmental disabilities
- provide relief both in and out of the home
- individuals under the age of 18 require a CWSDS application form to be completed
- individuals over the age of 18 require a Developmental Services Ontario (DSO) package * to determine eligibility for services and complete an assessment prior to a referral to an agency such as CWSDS, contact DSO at 1-888-941-1121
