Service Description: A mobile team made up of Registered Nurses, Registered Practical Nurses and Personal Support Workers available to long-term care homes

The team is trained and experienced to serve residents through episodes of responsive behaviours

The team will:
  • support and work with long-term care home staff to complete assessments and screening tools
  • collaborate with residents, their families and care team members to develop and implement strategies to meet resident needs
  • work with other organizations to help residents transition to and from their long-term care home
  • share knowledge to enhance staff capacity to serve residents



Fees: None
Application: Referral for service is made through the long-term care home
Eligibility / Target Population: Older adults residing in a long-term care home with cognitive impairments due to mental health problems, addictions, dementia, or other neurological conditions who exhibit or are at risk of exhibiting responsive or challenging behaviours such as aggression, wandering, physical resistance and agitation
Languages: English
Language Notes: French interpretation available upon request
