Service Description: Coordinates Non-Insured Health Benefit (NIHB) Medical Transportation services for people to attend medical appointments at Six Nations and in surrounding communities

Application: Call or visit * valid health card and certificate of Indian status required * members of the Six Nations of the Grand River must request transportation at least five days prior to appointment date * members of the Mississaugas of the Credit First Nation and other First Nations require a prior approval and must make request at least two weeks prior to appointment
