Application: Medical referral required by doctor or nurse practitioner
Program intake form completed on first contact with client, client must be the one to apply for the program and not the spouse
Eligibility / Target Population: Métis adults 55 years and older or self-identifying
First Nations/Inuit qualify for short-term services
