Service Description:
|
16-week homecare program that offers an individualized service plan whereby individuals receive at least two (but can be more) of the following types of services: personal support worker, occupational therapist, physical therapist, nursing, social worker, and registered dietitian * services are provided through Bayshore for patients living in St Catherines and Thorold and SE Health for patients living in south Niagara

Patients who are medically appropriate for discharge who require 16 weeks or so of homecare supports to be able to meet their medical, personal care and rehabilitative need
