Service Description: Rehabilitation, wellness programs, and community resource connections to help older adults stay safe and independent in their own home * focuses on helping older adults obtain optimal mobility, function, and independence * combines both ambulatory rehabilitation and wellness programming up to 12 weeks

Application: Medical referral required for ambulatory rehabilitation
Eligibility / Target Population: Ontario residents 60 years and older who are medically stable * must have a valid OHIP card * must be able to follow instructions and self-determine modifications suitable to personal health limitations * must be able to engage in conversation and make decisions independently * must be be able to transfer independently or with one person assisting
