Service Description:
|
Supports for retirement home residences with cognitive impairments and responsive behaviours * works with clients, families, residence staff, and community partners to:
- develop strategies to management responsive behaviours
- create a care plan
- refer clients to other services as appropriate
- identify educational opportunities for residence staff

Eligibility / Target Population:
|
Residents of select retirement homes in Hamilton who have cognitive impairments due to mental health problems, addictions, dementia or other neurological conditions and are exhibiting responsive behaviours such as aggression, wandering, physical resistance and agitation
|
