Service Description: Provides coordinated support to help patients transition safely from hospital to home, receiving follow-up care, education, and resources required for a smooth and confident recovery.
8 or 16 week restorative, individualized support, delivering at least two services such as personal support (PSW), therapy (PT, OT), nursing, social work, speech-language pathology, or dietitian care. Service delivery is region-based supporting residents of Niagara North and Niagara South.

Fees: Covered by OHIP
Application: Referral initiated through NH Hospital (must be an inpatient)
Eligibility / Target Population: Patients who are medically appropriate for discharge who require 8/16 weeks or so of homecare supports to be able to meet their medical, personal care and rehabilitative need
