01 - Physical Activities Promotion - Burlington


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP identifies people who engage in low levels of physical activity (fewer than two hours overs a three-day period). Assessors work with them to design a physical activity that matches their lifestyle.
