08 - Communication - Hamilton

Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on the connection between the person's communication abilities and their cognitive skills for daily decision-making. The goal is to work with the person to improve their communication ability whenever possible and to prevent avoidable communication decline.

See Deafness and Impaired Hearing, Hearing Aids and Communication Devices, Speech and Language, Speech Language Pathology - Adult Aphasia, and Speech Language Pathology - Adult Swallowing Clinic for care options for people who trigger this CAP.
