14 - Falls - Burlington


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on people at greater risk of falling in the future based on past falls. Assessors work to identify and change underlying risk factors for falls.
