12 - Informal Support - Niagara North West


Upon completion of interRAI Home Care Assessment, Client Assessment Protocols (CAPs) are triggered. The CAPs are intended to assist with focusing care planning around key issues identified during the assessment process.

This CAP focuses on situations in which a person will need help from others, looking to identify situations where organizations may have to step in to provide support. This CAP identifies people whose informal support systems (family, friends, neighbours) may not be fully meeting their needs.
