Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible

Dementia Care
  • one-on-one caregiver support
  • caregiver education sessions
  • adult day programs (Burlington, Oakville and Mississauga)
  • memory care (respite, home safety assessment, on-going assessment of care needs)
  • memory visiting (trained volunteers provide companionship, some respite)
  • respite care (overnight and private respite)

Home Care

  • nursing care (visiting, shift, specialty, and clinics)
  • personal support (help with bathing and showering, dressing, medication supervision, skin care, exercises, etc.)
  • overnight care
  • housekeeping and laundry
  • footcare (with fees)

Hospice Care and Bereavement Support - Emotional, social and spiritual support, companionship, respite care for people and their family/caregivers dealing with a life-limiting illness * services provided at home

Tele-Touch - A telephone reassurance program where volunteer callers phone lonely, isolated or frail older adults, providing friendly conversation, security and reassurance * volunteers call according to a pre-arranged schedule * if the volunteer is unable to reach the client, the agency will follow up until we establish that the client is safe and sound * volunteer opportunities available (apply online) * also provides security check-in calls
